The instrument scored with the scale: N/A, Not Done, Below Standard, Competent, Performed Independently, Required Assistance. Instrument may be implemented by the faculty / attending surgeon or as a 360 deg evaluation by a clinical team. The instrument includes a weighted scale for measuring the case difficulty level.
